Claim Form Instructions

Attorneys, interpreters and experts who provide legal services for indigent defendants are required to submit claim forms to receive compensation. Supreme Court Rule 13 governs the guidelines for compensation from the indigent defense fund. Please read the compensation instructions for information about compensation and the filing process.

Pursuant to an order filed February 6, 2013,  that effective July 1, 2013, the AOC will no longer accept paper claims from attorneys and interpreters seeking reimbursement from the Indigent Defense Fund.  Claims must be submitted on-line via the ICE system.   Experts, investigators and other support service providers will continue to submit paper claims.
 
To submit a paper claim, please download and complete the appropriate claim form. You cannot submit the paper forms on-line. They must be printed on your printer and mailed or hand delivered to appropriate office as indicated on the form. If you wish, you may fill them out as presented online, or you may right click on the link and save the file to your system. These forms perform all totaling and other calculations automatically. In order to take advantage of this feature, you must enter all information and then print out your form to be sent to the AOC.
 

Secure, by means of a staple, all pages of these forms, together with any required attachments before mailing them to the Administrative Office of the Courts. Failure to do so will delay the processing of your claim!
